Enter Damage Claims, track the progress of the claim. This project is in beta testing so we want feedback. If we have enough interest we'll add in features as they are requested.

A few things to add:
Vehicle Mileage
When was vehicle purchased Month/Year

Our Customer Assistance form has space for three separate vehicles in case there was a collision in the tunnel. Maybe we can choose to link several of these forms together if that happens.

Overall, it is a great start. I look forward to using this (hopefully not very often).

Looks great! I haven't had to use it yet, I guess thats a good thing! But when I do I will be using it!

Mileage would be ok...better would be "condition of item broken" (any indication of tape, glue, missing screws, alterations or scratches from previous damage)

We need to have a by site report by the year. We also need a button to email the report or modified report to an email address (office or repair shop)

Couldn't mileage, condition etc all be in incident description? The only reason I would want to have their own fields was to make the data more robust. The alternative is a bloated extra long form.

Yes...
what we need on the final version are fields that print on the form and are visible for the person completing the form, that we can establish/modify for our operations
IE: car wash name, location, liability disclaimer, instructions for description...ie: mileage, previous condition, etc, etc) instructions for repairs or estimates...etc, etc)

I think this is a great start. Damages are very difficult situations and many take a long time to correct or complete. The more professional we act and appear, the better.

I prefer my attendants just document them and I make final decisions. I or my maintenance man will often get the parts and install them at their home or place of business to make it more convenient for them.

IE: Liability disclaimer...Possesion of this form does not mean Speedy Car Wash assumes complete responsibility for this incident. Please do not repair your damage and expect compensation without approval.
